Job summary

Boost Home Healthcare in Michigan is seeking a licensed Physical Therapist to provide physician-prescribed therapy in patients' homes. The role focuses on improving function, promoting independence, and coordinating with the care team under the direction of Clinical Services.

Requirements include a degree in physical therapy (Bachelors/Masters/Doctorate), current state licensure, and preferably 2 years of home health experience with Medicare OASIS knowledge.

Qualifications

• Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a PT program approved by boards.
• Currently licensed in the state of practice.
• Two years’ experience preferred in home health.
• Knowledgeable with Medicare OASIS documentation.

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities.
• Return the patient to optimum level within capabilities.
• Participate with other home care personnel in patient care planning.
• Direct and supervise personnel as required.
• Initial assessment, plan of care, and 30-day reassessments.
