Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) for full-time home health visits in Florida. Work entails assessing mobility and function, creating plans of care, and delivering PT services in patients' homes. Strong documentation and coordination with care teams are essential.

Requirements include a PT degree, active state license, and CPR. Prior home health experience and OASIS familiarity are a plus.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in the state of practice.
• CPR certification preferred.
• Experience in Home Health Therapy and completing OASIS documentation is a plus.

Responsibilities

• Perform initial and ongoing mobility, coordination, strength, and functional assessments.
• Provide written evaluation and plan of care; document progress and outcomes.
• Deliver PT services per physician orders and maintain plan of care.
• Discharge planning and home exercise instruction for patients and caregivers.
• Coordinate with healthcare team and document in accordance with policies.
