Job summary

CenterWell Home Health seeks a Home Health Physical Therapist to help patients regain strength and independence through in-home care, reporting to the Clinical Manager. You will plan and deliver PT services in patients' homes.

The role emphasizes teamwork with clinicians and aides, documentation, and progress reporting. Requires license, CPR, and collaboration experience; preferred one year as PT. Compensation ranges from $93k to $128k per year with Humana benefits.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist (preferred).

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes to attain highest physical function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Provide clear care instructions to PTAs and Home Health Aides and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report progress to physicians, Clinical Manager, care team, patient and family; submit evaluations, plans and discharge summaries.
