Job summary

Choice Health at Home is seeking a compassionate Physical Therapist to join our home health team in Denver, CO. You will provide personalized therapy to improve mobility, relieve pain, and restore independence in patients’ homes.

This full-time role requires a PT license, CPR certification, and a valid driver’s license. Home health experience and Homecare Homebase familiarity are preferred. We offer comprehensive benefits and a patient-focused work environment.

Qualifications

• Graduate of an accredited physical therapy program.
• Current, valid Physical Therapist license.
• CPR certification.
• Valid driver’s license and current auto insurance.
• At least one year of home health experience preferred.
• Homecare Homebase experience preferred.

Responsibilities

• Perform and document comprehensive initial evaluations
• Identify functional problems and establish diagnoses before intervention
• Develop and implement individualized treatment plans
• Help patients maintain, improve, or restore physical function
• Provide treatments designed to alleviate pain and prevent further dysfunction
• Evaluate patient progress and adjust treatment plans to maximize benefit
• Follow infection-control standards and universal precautions
• Maintain accurate, timely documentation in accordance with company guidelines
• Ensure continuity and quality of patient care
• Communicate effectively with patients, families, team members, physicians, and other healthcare professionals
• Support Choice Health at Home’s philosophy, policies, and standards of care
