Vantage Health Partners in Broomfield, CO is seeking a Home Health Physical Therapist for a full-time, on-site role delivering rehabilitation directly in patients' homes and in assisted living facilities. You will evaluate patients, develop individualized care plans, and provide therapeutic exercise, mobility, strength, balance and coordination interventions while monitoring progress and documenting care.

• BS, MS, MPT, or DPT in Physical Therapy from an accredited program.
• Current Colorado Physical Therapist license.
• Proficiency in patient evaluation and care planning.
• Expertise in therapeutic exercise and mobility training.
• Skill in strength, balance and coordination interventions.
• Ability to monitor progress and produce thorough documentation.
• Preferred: at least one year of physical therapist experience.
• Preferred: home health or assisted living experience.
• Preferred: wellness program design experience.
• Preferred: interdisciplinary collaboration experience.
• Preferred: strong communication and interpersonal skills.

Responsabilidades

• Conduct comprehensive patient evaluations in home health and assisted living settings.
• Develop and implement individualized physical therapy care plans.
• Deliver therapeutic exercise and mobility training to restore function.
• Design and perform strength, balance, and coordination interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate, timely, and compliant clinical documentation.
• Educate patients and caregivers on safe home exercise and fall prevention.
• Collaborate with physicians, nurses, and other interdisciplinary team members.
• Support wellness program design and implementation where applicable.
• Travel across Broomfield, Longmont, and Boulder service areas.
