Job summary

UnitedHealth Group is seeking a Physical Therapist for home health to assess patients and determine treatment plans focused on functional status, mobility, endurance, and pain. You will implement interventions to improve well-being and evaluate progress, working with a care team in patients' homes.

Responsibilities include evaluating patients, developing plans of care, coordinating with physicians and nurses, and ensuring documentation and compliance with state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to dependable vehicle or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs and assists in development of the therapy plan of care.
• Provides PT services within scope of practice and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising staff.
• Treats patients to relieve pain, improve function, and maintain maximum functioning per standards of practice.
• Instructs patient, family, and care team about the plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
