Job summary

You will document progress, communicate with physicians and family, and ensure care plans align with physician orders while maintaining safety and compliance in a home-health setting.

Qualifications

• Licensed Physical Therapist with CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Ability to deliver care in patients' homes.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to restore function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop rehabilitation programs aligned with physician plans and patient goals.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document observations, interventions, and evaluations promptly.
• Report progress to physicians, Clinical Manager, care team, patient, and family.
