Job summary

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to restore function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop rehab programs aligned with physician plans and team goals.
• Coordinate with PTAs and Home Health Aides and document care.
• Record observations and evaluations promptly and thoroughly.
• Communicate patient progress to physician, care team, patient, and family.
