Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to work in patients' homes, reporting to the Clinical Manager. You will plan and administer prescribed therapy to improve function and independence, documenting progress for physician review and care team communications.

You will assess strengths, develop rehab plans, supervise PTAs and aides, and ensure timely documentation. This patient-facing role includes driving requirements and comprehensive benefits.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes.
• Assess and document patients' physical strengths to guide goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide clear written instructions to PTAs and Home Health Aides.
• Document observations, interventions and evaluations promptly.
• Report progress to physician, Clinical Manager and care team.
