Job summary

CenterWell Home Health is seeking a devoted Home Health Physical Therapist to help patients regain strength, mobility and independence in the comfort of their homes, reporting to the Clinical Manager.

You will plan, administer, assess and document physical therapy interventions, collaborating with a multidisciplinary team to align care with the physician's Plan of Treatment and patient goals, while ensuring high-quality outcomes.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop rehabilitation programs aligned with the Plan of Treatment and patient goals.
• Provide written care instructions to PTAs and Home Health Aides and oversee performance.
• Document observations, interventions, and evaluations promptly.
• Report progress to physicians, Clinical Manager, care team, patient, and family; submit required documents.
