Job summary

Interim HealthCare of Queen Creek & Gilbert is seeking a Home Health Physical Therapist to provide in-home care across Queen Creek, Chandler, and surrounding East Valley communities. The role emphasizes autonomy, clinical judgment, and measurable progress toward patient goals.

The PT will evaluate patients at home, develop plans of care, deliver therapeutic exercises, and educate families while coordinating with the care team.

Qualifications

• Graduate of an accredited physical therapy program with an active Arizona PT license.
• Minimum of 1 year of physical therapy experience.
• Home health experience preferred; CPR certification required.

Responsibilities

• Evaluate mobility, strength, balance, pain, and functional limitations in the home environment.
• Develop and update individualized therapy plans aligned with physician orders and patient goals.
• Provide therapeutic exercise, hands-on interventions, gait training, and mobility instruction.
• Recommend and train patients on appropriate assistive devices and safety strategies.
• Educate patients and caregivers on exercises, fall prevention, and safe techniques.
• Track progress, adjust interventions, and maintain timely, accurate documentation.
• Collaborate with nursing, occupational therapy, speech therapy, and other care team members.
