Job summary

Place 2B Properties in Chandler, AZ is seeking a Physical Therapist or Occupational Therapist to assess, plan, and deliver in-home therapy programs focused on improving mobility and daily living skills.

You will train caregivers, document progress, collaborate with physicians and the care team, and ensure compliance with healthcare regulations while working a travel-heavy schedule. This role provides the opportunity to adapt care plans to individual environments and promote independence.

Qualifications

• Accredited Degree in Physical Therapy (PT) or Occupational Therapy (OT) with an active State License.
• CPR/First Aid certification; previous home health or clinical experience preferred.
• Ability to travel daily (including nights/weekends as needed) and lift up to 50 lbs.

Responsibilities

• Assessments & Plans: Conduct therapy evaluations and build tailored treatment plans based on client goals.
• Therapy & Training: Deliver rehabilitation exercises, train clients/caregivers on adaptive equipment, and promote safety.
• Care Coordination: Document progress, update care plans, and collaborate with physicians, nurses, and the care team.
• Compliance: Maintain timely, accurate clinical records in line with state/federal healthcare regulations.
