Job summary

Interim HealthCare of Queen Creek is hiring a Home Health Physical Therapist (PT) to support patients across Queen Creek, Chandler, Mesa, Tempe and surrounding East Valley communities. This per diem role emphasizes autonomy, one-on-one care, and goals-based therapy delivered in patients’ homes.

You will evaluate mobility, develop plans of care, provide exercise and gait training, educate patients and caregivers, and collaborate with the care team to track progress and ensure safe, effective

Qualifications

• Graduate of an accredited physical therapy program with an active Arizona PT license.
• Minimum of 1 year of physical therapy experience.
• Home health experience preferred.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Strong clinical judgment, communication, organization, and documentation skills.

Responsibilities

• Evaluate mobility, strength, balance, pain, and functional limitations in the home environment
• Develop and update individualized therapy plans aligned with physician orders and patient goals
• Provide therapeutic exercise, hands-on interventions, gait training, and mobility instruction
• Recommend and train patients on appropriate assistive devices and safety strategies
• Educate patients and caregivers on exercises, fall prevention, and safe techniques
• Track progress, adjust interventions, and maintain timely, accurate documentation
• Collaborate with nursing, occupational therapy, speech therapy, and other care team members
