Job summary

Interim HealthCare of Queen Creek & Gilbert is seeking a Home Health Physical Therapist to deliver personalized therapy in patients' homes across the East Valley. You will assess mobility and balance, design therapy plans, and guide patients toward functional independence.

The role offers per diem scheduling, one-on-one patient care, and opportunities for professional growth within a supportive team.

Qualifications

• Graduate of an accredited physical therapy program with an active Arizona PT license.
• Home health experience preferred.
• CPR certification may be required.

Responsibilities

• Evaluate mobility, strength, balance and functional limitations in the home.
• Develop and update individualized therapy plans per physician orders.
• Provide therapeutic exercise, gait training and mobility instruction.
• Educate patients and caregivers on exercises, safety and fall prevention.
• Track progress and maintain timely documentation; collaborate with care team.
