Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our home health team in Wynantskill, NY. You will deliver individualized, evidence-based therapy to patients in their homes, enjoying schedule flexibility and a supportive team environment.

Responsibilities include evaluating patients, developing plans, implementing interventions, and maintaining documentation for regulatory compliance. A NY PT license is required, with home visit experience preferred.

Qualifications

• Active NY Physical Therapy License (PTA) required.
• New York license to practice Physical Therapy is required.
• Proficient in electronic documentation and basic computer use.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide evidence-based PT interventions to improve motor skills and independence.
• Maintain accurate documentation of patient care.
• Collaborate with the clinical team to ensure coordinated treatment.
• Communicate progress and care plans to patients and caregivers.
