Job summary

Homeside Rehab is seeking a compassionate Physical Therapist or PTA to join our home health team in Manorville, NY. You will deliver individualized, evidence-based therapy to patients in their homes, offering schedule flexibility and a supportive team environment.

Responsibilities include evaluating patients, developing treatment plans, and delivering interventions to improve motor skills and functional independence. NY licensed PT/PTA required; ability to travel to patient homes is essential.

Qualifications

• Active NY Physical Therapist / Physical Therapist Assistant License (PT/PTA) is required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals.
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes.
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans.
