Job summary

BrightStar Care is seeking a licensed Physical Therapist (PT) for in-home visits in Longmeadow, MA. This part-time / per diem role offers flexible scheduling and mileage reimbursement.

You will perform in-home PT evaluations, create personalized care plans, provide skilled interventions to improve strength, balance, mobility, and safety, and educate clients and families on exercises and home safety.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program required.
• CPR certification required.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
