Job summary

The Christ Hospital Home Health Care is seeking a Physical Therapist to provide skilled in-home therapy, develop and adjust plans of care, and ensure safe, effective treatment for patients in the Cincinnati area. This role requires a current PT license, acute PT experience, and a valid driver’s license to travel to patient homes.

You will educate patients and families, document progress, and collaborate with physicians and agency staff to optimize care and outcomes.

Qualifications

• Minimum of one year of PT experience in an acute setting.
• Home care experience is desired.
• Physical Therapist license in the state of employment is required.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled physical therapy in patients' homes as ordered by the physician.
• Observe, record, and report patient progress to physician and agency staff.
• Develop and adjust plan of care based on patient needs and OASIS assessments where applicable.
• Promote continuity of care with admissions, transfers and discharges.
• Instruct patients and families on home exercise and use of equipment; educate on disease/injury management.
