Job summary

The Christ Hospital Home Health Care is seeking a licensed Physical Therapist Assistant to deliver skilled services in patients’ homes under a Physical Therapist’s direction. This PRN position covers a service area including East Hamilton, Oxford, Ross, Fairfield, and surrounding parts of Cincinnati.

The PTA educates patients and families, documents care, and follows state guidelines for licensed PTA practice. A caring team supports scheduling, admissions, and patient transitions home.

Qualifications

• Physical Therapist Assistant license current in employment state.
• Minimum one year PTA experience in an acute setting required.
• Home care experience is a plus.
• Ability to communicate clearly with patients, families, physicians and staff.
• Knowledge of therapy best practices and accurate clinical judgment.
• Detail oriented with strong organizational skills.
• Must have valid driver’s license and auto insurance.

Responsibilities

• Provide skilled PTA services in patients' homes per plan of care.
• Observe, document, and report patient changes and progress.
• Perform PT procedures prescribed by the supervising PT.
• Educate patients and families on therapy techniques and safety.
• Complete all clinical documentation per agency and Medicare guidelines.
