Job summary

Mercy is seeking an in-home physical therapist to provide treatment to patients with neuro and/or musculoskeletal disorders under physician supervision. The role focuses on delivering patient-centered care in home settings and coordinating with physicians and care teams.

The position requires graduation from an accredited physical therapy program, current state license, CPR certification, and a valid driver's license with the ability to lift up to 50 pounds.

Qualifications

• Graduate of an accredited physical therapy program.
• Current state license and CPR certification.
• Valid driver’s license and ability to lift up to 50 pounds regularly.
• Strong assessment, treatment planning, communication, and documentation skills.
• Good physical health, problem-solving, time management, and adaptability.
• Preferred: Two years of experience in acute or home care.

Responsibilities

• Assess, plan, and implement individualized treatment plans.
• Schedule patient visits independently, ensuring timely care.
• Maintain documentation and communicate effectively with team members, physicians, and patients.
• Handle emergencies and adapt to changing circumstances independently.
• Drive safely in various conditions, following instructions and maps as needed.
• Participate in weekend and on-call rotations.
