Job summary

Advocate Health is expanding a new at-home outpatient rehabilitation program serving the Northern Charlotte area. The role provides high-quality, one-on-one physical therapy in patients' homes with autonomy to tailor plans and collaborate with physicians and caregivers.

You will manage a defined geographic caseload, utilize technology for scheduling and outcomes, and supervise support staff as required for licensure. This full-time position emphasizes access and innovation in care delivery.

Qualifications

• Requires a Bachelor’s degree in Physical Therapy and state licensure.
• BLS certification required per policy guidelines.
• Experience in outpatient or home-based rehabilitation preferred.

Responsibilities

• Deliver high-quality, one-on-one outpatient physical therapy in patients' homes.
• Provide care equivalent to clinic services while improving access and reducing transportation barriers.
• Develop individualized treatment plans and collaborate with physicians and caregivers.
• Manage a caseload across a defined geographic area with evidence-based interventions.
• Coordinate scheduling, progress updates, and discharge planning with the care team.
• Supervise PTA’s and Rehab Tech/Aide as required for licensure.
