Job summary

Advocate Health is launching a new at-home outpatient rehabilitation program in the Northern Charlotte area. We seek a licensed physical therapist to deliver one-on-one services in patients' homes, focusing on mobility, function, and independence.

You will develop individualized plans, collaborate with physicians and caregivers, and use flexible scheduling to serve a defined geographic area while maintaining outcomes and satisfaction.

Qualifications

• Bachelor's degree in Physical Therapy required.
• State license to practice physical therapy required.
• BLS certification required per policy.

Responsibilities

• Deliver high-quality, one-on-one outpatient physical therapy services in patients' homes.
• Provide the same evidence-based outpatient care as clinic-based settings.
• Develop individualized treatment plans and collaborate with physicians, caregivers, and team members.
• Utilize technology and flexible scheduling to manage a caseload across a geographic area.
• Join an innovative care model combining clinical excellence with care delivered at patients' homes.
