Job summary

Powerback Rehabilitation is seeking a Physical Therapist to deliver outpatient rehabilitation services primarily in home settings for older adults in Lynn, MA. The role emphasizes evaluating, designing, and delivering PT programs to restore function and independence in clients’ homes or in assisted living facilities.

The position offers a flexible schedule, mentorship for new grads, and opportunities for CEUs and professional growth through the Powerback programs.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy (or equivalent)
• Bachelor's degree in Physical Therapy plus 5 years of PT experience acceptable as alternative.

Responsibilities

• Assess and treat patients in home and community settings.
• Develop and implement individualized PT plans of care.
• Supervise assistants, trainees, and staff as needed.
• Coordinate with rehab team to address comprehensive patient needs.
• Document treatment and progress in medical records.
• Promote safety and educate patients and caregivers.
