Job summary

FOX Rehabilitation in Red Bank, NJ, is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. You will evaluate, plan, and implement individualized therapy, promoting mobility, independence, and quality of life through our Geriatric House Calls model.

Roles include in-home visits, some telehealth, and the opportunity to build lasting patient relationships.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility and independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
