Job summary

FOX Rehabilitation in Middletown, NJ seeks licensed Physical Therapists to deliver one-on-one geriatric care in homes or senior living communities. You will assess, plan, and treat to improve mobility, independence, and quality of life.

Qualifications

• Valid Physical Therapy license in NJ or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer skills and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility and independence.
• Document in FOX's EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
