Job summary

Senior Care Solutions is seeking a compassionate Physical Therapist to join our Medicare-certified home health team as a Baylor PRN weekend provider. You will deliver skilled therapy in patients' homes, focusing on mobility, strength, and overall quality of life.

The role requires a Florida license, at least one year of PT experience, and the ability to work autonomously with a flexible weekend schedule. Documentation and compliance with agency standards are essential.

Qualifications

• Active Florida Physical Therapy license.
• Minimum 1 year of PT experience.
• Home health experience preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage field-based schedule.
• CPR certification required.

Responsibilities

• Perform comprehensive PT evaluations (SOC, Recertification, Discharge).
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, gait and balance training, pain management, functional mobility.
• Educate patients/caregivers on home exercise programs, safety and fall prevention.
• Complete EMR documentation within required timeframes.
• Collaborate with physicians, nurses, therapists, and the care team.
• Participate in case conferences and care coordination.
• Maintain Medicare and agency standards compliance.
