Qualifications

• Eligible New Jersey Physical Therapist license.
• Willing to travel to patients' homes.
• Per diem, 1099 independent contractor arrangement.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment and develop individualized plans.
• Evaluate patients and align with physician orders and patient needs.
• Deliver care per professional standards and agency policies.
• Educate family members and caregivers on therapy techniques.
• Plan and carry out therapy programs to improve function and relieve pain.
• Perform initial exams and determine diagnoses prior to intervention.
• Provide timely, accurate documentation for each visit.
• Evaluate treatment effects and adjust accordingly.
• Willingness to treat geriatric patients.
