Qualifications

• Licensed to practice as a Physical Therapist in Iowa.
• Minimum of two years of physical therapy experience.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations.
• Develop and implement individualized treatment plans focusing on mobility, strength, balance, and safety.
• Provide interventions including exercises, gait training, transfer training, and pain management.
• Educate patients and caregivers on home exercise programs, safety, and equipment use.
• Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care.
• Document all visits, progress notes, and care plans in compliance with standards.
• Recommend adaptive equipment and home modifications when appropriate.
• Monitor progress and adjust treatment plans as needed.
• Communicate changes in condition to the care team.
• Promote independence and ongoing improvement through patient-centered care.
