Job summary

FOX Rehabilitation in Ridgefield, CT seeks licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. You’ll create and manage treatment plans, perform evaluations, and help patients improve mobility and independence in a flexible schedule.

Clinicians collaborate with PTs, OTs, SLPs, and Fitness Specialists, with options for telehealth and in-home visits. Competitive benefits and professional growth are offered.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
