Job summary

Fox Rehabilitation is expanding in Brookfield, CT, seeking licensed Physical Therapists to deliver one-on-one care to older adults in private homes and senior living communities. You will evaluate, plan, and implement individualized care while maintaining autonomy over your schedule.

Qualifications

• Must have a degree from an accredited Physical Therapy program.
• Must hold a valid Physical Therapy license in CT or be eligible to obtain.
• Current CPR certification is required.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and functional goals.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
