FOX Rehabilitation is expanding in Windsor, CT, offering licensed Physical Therapists in-home and in senior living settings. You will deliver one-on-one geriatric PT care, evaluate patients, and create individualized plans of care that improve mobility and independence.

Competenze

• Valid Physical Therapy license in CT or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Mansioni

• Provide one-on-one physical therapy to older adults in homes and communities
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility, and independence
• Document in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access
