Interim HealthCare Inc. is seeking a Home Health Physical Therapist - PT to provide compassionate, one-on-one therapy in patients' homes across Dublin, Radford, and surrounding areas. This role emphasizes independence, personalized treatment plans, and patient-centered care.

The position offers flexible scheduling, potential for full- or part-time hours, and a supportive multidisciplinary team focused on improving mobility and quality of life for home-bound patients.

Qualifications

• Graduate of an accredited Physical Therapist program.
• Active PT license in the state of Virginia.
• Minimum of 2 years of physical therapy experience (home health preferred).
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, interpersonal, and critical thinking skills.
• Compassionate, dependable, and patient-focused.
• Ability to work independently in a home setting.
• Willingness to travel throughout the service area.

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on therapy goals, exercises, and self-care techniques
• Coach family members on safely assisting with exercises and patient care
• Collaborate with interdisciplinary team including RN, LPN, CNA, HHA, OT, and SLP
• Maintain accurate and timely documentation of patient progress
