Job summary

Interim HealthCare Inc. is seeking a Home Health Physical Therapist - PT to provide one-on-one therapy in Lynchburg, VA, delivering care in patients' homes and helping them regain strength and mobility.

The role requires Virginia-licensed PT, 2+ years experience (home health preferred), CPR, and willingness to travel across Lynchburg and surrounding areas with a flexible FT/PRN schedule and mileage reimbursement.

Qualifications

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification and TB baseline screening
• Strong clinical judgment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently in a home setting
• Willingness to travel throughout the service area

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct initial assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on their plan of care, therapy goals, and self-care techniques
• Coach family members on how to safely assist with care and exercises
• Collaborate with interdisciplinary team members (RN, LPN, CNA, OT, SLP)
• Maintain accurate and timely documentation
