All Care and Comcare Home Health is seeking a compassionate and motivated Physical Therapist to join our growing team. This field role offers the opportunity to deliver high-quality, individualized care in patients' homes while enjoying a flexible schedule and supportive team environment.

As a Field Physical Therapist in California, you will perform SOC/ROC/Recert/DC evaluations, collaborate with an interdisciplinary team, and educate patients and families on home exercise programs.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout the service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services in accordance with physician orders.
• Perform start of care (SOC), resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
