Job summary

The VIOS Clinic is seeking a Physical Therapist to provide outpatient rehabilitation services primarily in in-home settings for older adults, including private residences and independent living facilities.

You will assess patient needs, develop and implement personalized therapy programs, educate patients and caregivers, and monitor progress while collaborating with a team to ensure safety and quality of care.

Qualifications

• Valid license to practice physical therapy.
• Master's degree in physical therapy or equivalent.
• Reliable transportation to home-based settings.
• Experience with home-based or elderly care is preferred.

Responsibilities

• Assess patient needs and develop personalized therapy plans.
• Educate patients and caregivers to promote independence.
• Refer to other disciplines as necessary and monitor progress.
• Supervise support staff and ensure documentation compliance.
• Participate in team meetings and maintain equipment and safety standards.
