Job summary

Powerback is seeking a Physical Therapist to provide in-home outpatient rehab for older adults, in private homes or living facilities. You will assess needs, create and implement therapy plans, and educate patients and caregivers to promote independence.

Responsibilities include coordinating with the rehab team, supervising support staff, maintaining documentation, and adapting plans as needed. The role offers a flexible schedule across home and community settings with growth opportunities.

Qualifications

• Must hold a valid Physical Therapy license in practice states.
• Master’s degree in Physical Therapy or equivalent combined with experience.
• Experience delivering in-home rehabilitation and patient education.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Develop and deliver individualized physical therapy plans.
• Educate patients and caregivers to promote independence.
• Refer to other disciplines as needed and coordinate care.
• Supervise PT assistants and ensure documentation accuracy.
• Maintain a safe home settings and manage equipment.
