Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Canyon Lake, TX. You will evaluate patients in their homes, develop treatment plans, and guide gait training, manual therapy, and balance retraining to improve mobility and reduce pain.

We value patient-centered care, strong communication, and teamwork. A TX PT license and DPT are required, with home health experience preferred.

Qualifications

• Active TX Physical Therapist license is required.
• DPT degree or higher from an accredited program.
• Experience in home health or rehabilitative settings preferred.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Document evaluations, sessions, and progress notes in digital platforms.
• Collaborate with physicians, nurses, and occupational therapists to coordinate care.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate, patient-centered care to improve independence.
