Qualifications

• Licensed Physical Therapist with active CA license.
• Minimum 1 year of licensed PT experience.
• Reliable communication and timely documentation.
• Valid driver’s license and transportation.
• CPR certification and professional liability insurance.
• Recent physical examination and TB test as required.

Responsibilities

• Provide 45 minutes of rehabilitation services in patients' homes.
• Assess patients’ physical condition and develop personalized plans.
• Deliver hands-on therapy and monitor progress.
• Assist patients to achieve goals and improve quality of life.
