Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide in-home therapy, delivering high-quality, individualized care in patients' homes with a flexible schedule and supportive team environment.

The Field Physical Therapist will perform SOC evaluations, develop treatment plans, coordinate with the care team, and educate patients and families on home exercise programs. Travel to patient homes is required across our service area.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform SOC evaluations, ROC, recertifications, and discharges.
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on therapy programs and home exercises.
