Job summary

Uplift HomeCare seeks a compassionate Physical Therapist to provide in-home therapy across Winter Haven, FL and surrounding areas. This field-based role offers PRN/part-time opportunities with flexible scheduling and competitive pay, enabling you to help seniors remain mobile and safe at home.

You will evaluate patients, design individualized treatment plans, and collaborate with a care team to ensure quality, HIPAA-compliant patient care while maintaining documentation and regulatory standards.

Qualifications

• Current, unrestricted Florida Physical Therapist license.
• Home health experience preferred; requirements may vary by assignment.

Responsibilities

• Evaluate patients' physical abilities, mobility, strength, balance, coordination, and functional limitations.
• Develop and implement individualized PT treatment plans based on patient needs and goals.
• Provide therapeutic exercises, gait, balance, transfer, and mobility interventions.
• Assess patient progress and adjust treatment plans accordingly.
• Educate patients and caregivers on home exercise programs, fall prevention, safe mobility, and use of assistive devices.
