Job summary

Uplift HomeCare LLC seeks a compassionate Physical Therapist to provide in-home therapy across Bradenton and surrounding areas in a field-based, PRN/part-time role with flexible scheduling and competitive pay. You will help seniors improve mobility and safety in their own homes.

The ideal candidate holds an unrestricted Florida PT license, has home health experience, and can coordinate care with a multidisciplinary team while documenting in EMR systems.

Qualifications

• Current, unrestricted Florida PT license.
• Home health experience preferred.
• CPR certification required.
• AHCA Level II background screening required.
• Valid driver's license and reliable transportation.
• Strong clinical and time-management skills.

Responsibilities

• Evaluate patients' mobility, strength, balance, and functional abilities.
• Develop and implement individualized home-based PT treatment plans.
• Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.
• Monitor progress and adjust plans as needed.
• Educate patients and caregivers on home exercise programs, fall prevention, safety, and use of assistive devices.
• Coordinate with physicians, nurses, OT, and other team members to ensure comprehensive care.
• Complete PT evaluations, reassessments, progress notes, discharge summaries, and required documentation on time.
