Job summary

THHN Consulting LLC is seeking a licensed Physical Therapist to subcontract home visits in the Bradenton, FL area. You will assess patients, design treatment plans, and deliver hands-on therapy in patients' homes, collaborating with physicians and caregivers.

Flexible scheduling, independence in choosing caseloads, and support from a therapist-first team. This role emphasizes quality patient care and professional liability, with per-visit pay and ongoing onboarding support.

Qualifications

• Active Florida license in Physical Therapy or eligible for licensure.
• DPT or equivalent degree with active PT license in good standing.
• Current CPR/BLS certification (BLS for Healthcare Providers).
• Proof of professional liability insurance.
• Valid W-9 provided.
• Reliable transportation for home visits.
• Strong documentation skills and attention to clinical detail.

Responsibilities

• Conduct comprehensive initial evaluations and reassessments in patients' homes.
• Develop and implement individualized treatment plans based on physician referrals.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Maintain accurate and timely clinical documentation per Medicare and payer guidelines.
• Coordinate care with physicians, nurses, and other therapy disciplines as needed.
