Job summary

CVHCare is seeking Licensed Physical Therapists to join our California Home Health team. You will evaluate mobility, set goals, and deliver therapy in patients' homes with independence guided by our supportive culture.

Entry level welcomed; full- or part-time schedules available across Livermore, San Ramon, Walnut Creek area. Benefits include health/dental/vision, 401(k), PTO, and mileage reimbursement.

Qualifications

• Minimum one year experience as Licensed Physical Therapist (preferred)
• Open to entry level or candidates with no prior home health experience
• Current/active Physical Therapy License – State of California
• BLS Certification
• Strong attention to detail for chart documentation
• Excellent time management/calendaring skills and ability to deliver documentation timely
• Excellent written and verbal communication

Responsibilities

• Evaluate mobility, strength and function; set goals to improve balance, walking and strength
• Assess home environment to ensure safe mobility and recommend equipment
• Educate patients on proper use of therapy techniques and home exercises
• Set own schedule within Home Health model with independence under company guidance
