Job summary

TalentBurst, Inc is seeking a local contract Physical Therapist for an outpatient/home health role in Albuquerque, New Mexico. The assignment offers 40 hours per week with a shift from 8:00 AM to 4:30 PM and a 13-week duration in a flexible home-based care environment.

The Physical Therapist will assess, plan, and implement therapy programs, educate patients and caregivers, and collaborate with a multidisciplinary team to promote independence and safe mobility in the home setting.

Qualifications

Must have a valid Physical Therapy license in the state of practice.
Master's degree in Physical Therapy (or equivalent).
Bachelor's degree in Physical Therapy with 5 years of PT experience or Master's + related field.
Driver's license or reliable transportation to patients' homes.

Responsibilities

Provide outpatient rehabilitation services in the home or facility setting.
Develop and deliver individualized therapy programs to restore and enhance function.
educate patients and caregivers; coordinate with rehabilitation team for comprehensive care.
Screen, examine, and document patient progress; adapt plan of care as needed.
Order supplies, ensure safety, and maintain equipment and documentation.
