Job summary

Optimal Home Care is seeking a Physical Therapist to provide home-based services in Colorado. The role emphasizes patient safety, compliance with HIPAA, and collaboration with physicians to evaluate and treat clients in their homes.

The successful candidate will document progress, educate patients and families in PT techniques and assistive devices, and contribute to discharge planning and care coordination with the nursing team.

Qualifications

• Holds an active license to practice in Colorado.
• Preferrable at least one year of experience as a Physical Therapist.
• Possesses adequate and effective written and verbal communication skills.
• Possess adequate and effective interpersonal skills.

Responsibilities

• Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
• Assists the Physician in evaluating client/patients by applying diagnostic and prognostic muscle, nerve, joint and functional tests and participates in the Physician’s Plan of Care.
• Develops, documents, implements and re-evaluates a specific Plan of Care noting all treatment modalities and frequencies.
• Treats patients to relieve pain and/or develop or restore function to maintain maximum performance.
• Reports to the Physician the client/patient’s reaction to treatment and/or any changes in the client/patient’s condition.
• Records clinical/progress notes in the patient’s clinical record weekly. Instructs client/patients and their families in the use of prosthetic, orthotic and assistive devices.
• Instructs the family in the client/patient’s total physical therapy program and evaluates the home environment.
• Instructs other involved agency personnel in the care of client/patients receiving physical therapy.
• Plans for patient’s discharge, prepares discharge summaries and instructions.
