Job summary

Advanced-Health-Care-6 is seeking a Home Health Physical Therapist to deliver patient-centered therapy in clients' homes across the Denver area. You will evaluate, diagnose, and treat individuals with mobility impairments, developing personalized plans and coordinating with families and the healthcare team.

The position is full-time, weekdays, day shift, with no weekends. Applicants should hold a valid Physical Therapy license and have 2–5 years of experience in home health or similar settings,

Qualifications

• Valid Physical Therapy license in the state where work is performed.
• 2 - 5 years of physical therapy experience, preferably in a home health or similar setting is preferred.
• Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.
• Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
• Ability to build rapport with patients through a compassionate and caring demeanor.

Responsibilities

• Conduct thorough evaluations of patients’ physical abilities, limitations, and needs to develop individualized therapy plans.
• Develop and implement effective treatment programs to improve patients’ mobility, strength, flexibility, and overall functional performance.
• Utilize therapeutic exercises, manual therapy techniques, modalities, and assistive devices to address specific physical impairments and goals.
• Educate patients and their families on exercise techniques, home exercise programs and strategies to support recovery and enhance daily living activities.
• Monitor and document patient progress, responses to therapy, and any changes in condition providing detailed reports to the supervising physicians and other healthcare team members.
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated approach to patient care.
• Provide guidance on the use of assistive devices and home modifications to improve patient functional independence.
• Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
• Participate in the development of care plans and case conferences with the healthcare team to optimize patient outcomes.
• Tay currently has advancements in physical therapy practices and participates in ongoing professional development.
• Perform all other duties assigned.
