Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to help patients regain strength, mobility, and independence in the comfort of their homes. Reporting to the Clinical Manager, you will deliver personalized care focused on functional improvement and work with a collaborative care team.

You will plan and administer prescribed PT in patients' homes, assess progress, set rehabilitation goals, supervise PTAs and home health aides, and document evaluations and discharge

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed PT in patients' homes to attain optimal physical function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement a rehabilitation program aligned with the Plan of Treatment.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee documentation.
• Document patient observations, interventions, and evaluations promptly.
• Report progress to physicians, Clinical Manager, care team, patient and family; submit evaluations and discharge summaries.
