Responsibilities

• Develop individualized treatment plans aimed at improving ADLs.
• Deliver hands-on therapy and interventions in the patient’s home.
• Educate patients, families, and caregivers on mobility techniques.
• Track and evaluate patient outcomes, adjusting plans of care.
• Collaborate with interdisciplinary team members for coordinated care.
• Document all visits, treatments, and progress as per standards.
• Maintain reliable attendance to ensure continuity of care.
