CoxHealth is seeking a Licensed Physical Therapist to deliver in-home PT evaluations and treatments in Springfield, MO. You will help relieve pain, restore function, and maintain optimal performance for patients in a home health setting.

Responsibilities include patient assessments, plan of care development, and coordination with care teams. Requires a state license, valid driver's license, and reliable transportation for community visits. Full-time, day shift, 40 hours per week.

Qualifications

• Graduate from an approved Physical Therapy program.
• Current Physical Therapist license or state registration.
• Active driver's license and reliable transportation.

Responsibilities

• Provide in-home evaluation, direction, and application of Physical Therapy services.
• Relieve pain, develop or restore function, and maintain maximum physical performance.
• Meet participation requirements and ensure quality patient care in home settings.
