Innovive Health is seeking a compassionate Physical Therapist to join our home health team in Massachusetts. You will provide in-home evaluations, develop individualized treatment plans, and guide patients through therapy to restore independence.

The role emphasizes autonomy, flexibility, and close collaboration with interdisciplinary team members to ensure quality care and safe discharge planning.

Qualifications

• Licensed Physical Therapist in Massachusetts.
• Degree from an accredited Physical Therapy program.
• CPR certification (current).
• Valid driver's license and reliable transportation.
• Home health experience required.
• Strong communication, organization, and clinical decision-making skills.

Responsibilities

• Provide in-home physical therapy evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and discharges.
• Develop and implement personalized treatment plans based on patient needs and physician orders.
• Guide patients through exercises, gait training, balance and mobility activities.
• Educate patients and caregivers on safety, fall prevention, and daily living strategies.
• Monitor progress and adjust treatment plans to achieve outcomes.
• Collaborate with nurses, occupational therapists, speech therapists, and physicians.
• Complete timely documentation and support discharge planning.
